Each year, the Illinois Department of Public Health conducts surveys upon the nursing homes in the state.  Those surveys review nursing home policies, procedures, and practices to determine whether the nursing homes comply with state laws, as well as the rights guaranteed to all nursing home residents.

The following nursing homes received "Type A" citations.  According to Illinois regulations, a Type A citation is warranted when a condition or occurrence at the nursing home "creates a substantial probability that the risk of death or serious mental or physician harm will result therefrom or has resulted in actual physical or mental harm to a resident."
